Substance Abuse Evaluations

A substance abuse evaluation is conducted to determine whether an individual has a problematic pattern of substance use that could benefit from treatment. The evaluation includes an extensive clinical interview and the completion of several tests.

Adult and Adolescent Evaluations are conducted at Cornerstone, 195 Featherway, Suite 1, Evanston, Wyoming. The cost of the substance abuse evaluation is $140. A $70 deposit is required prior to making the appointment. If the client fails to attend the scheduled appointment, does not complete required paperwork, or fails to cancel or reschedule at least 48 hours before the appointment time, the $70 deposit will be forfeited. The remaining $70 is due at the time of the evaluation. Evaluations are appropriate for all referral sources. Evaluation time is approximately 3 to 3 ½ hours, 1 ½ hours for oral interview and 1 ½ to 2 hours testing.

Substance Abuse Professional (SAP) Evaluations are also available. The initial evaluation is $250. A $70 deposit is required prior to making the appointment. If the client fails to attend the scheduled appointment, does not complete required paperwork, or fails to cancel or reschedule at least 48 hours before the appointment time, the $70 deposit will be forfeited. The remaining $180 is due at the time of the evaluation. A fee of $75 is also required for the follow up evaluation.
